All new patients are required to fill a New Patient Medical and Dental History Form. While you can fill them out at our office, you can reduce your waiting time by completing them in the comfort of your own home.

If you would like to save time during your visit, please print and fill out these forms and bring them with you to your first appointment. To complete your registration, we ask that you bring a copy of your current insurance card and photo ID. We will work hand in hand with you to maximize your insurance reimbursement for covered procedures.

If you are transferring to our office from another dentist, please contact them and request copies of any “current” x-rays, you may have.

These easy steps will save you time during your visit, but please know that they are here only should you wish to complete. Our staff will be happy to work with you on any questions you may have regarding your upcoming appointment.
